When completing your purchase please include the following information in the notes field in order to complete your report:

1. Date of Birth – Please include Month, Day & Year
2. Time of Birth - If you know it, if you do not it’s okay!
3. Place of Birth
4. Favorite Number / Number Combination 

This is optional, but if you would like insight on why you favor a particular number and or have been noticing a particular number combination around you at this time, we can pull some insight as to what is the meaning behind it.
